The Increasing access to quality and inclusive diabetes care project will be implemented in Yogyakarta Province. The project is planned to be implemented in 3 districts/city; Sleman, Yogyakarta City and Gunungkidul. The project aims to strengthen the inclusiveness of quality, integrated diabetes services all along the continuum of care in line with SDG's Goal 3 'Leave No one Behind'

In order to implement the project goal, HI is currently looking for a Community Health Facilitator for its office in Yogyakarta. The post holder will be will be engaged in the implementation of the project ‘Increasing Access to quality and inclusive diabetes care project.   The detail is as follows:

Position Title: Community Health Facilitator (CHF)
Location: Yogyakarta
Closing date: rolling (this position will be filled as soon as the suitable candidate is identified)
Basic Salary Range: Rp 4 – 4.8 million IDR depending on objective weighting of candidate’s profile and non-negotiable.

The successful candidate is expected to start as soon as possible early October 2019.

TASKS/RESPONSIBILITIES:

Under the line management of the Project Manager (PM), the Community Health Facilitator will support the Public Health Officer in the preparatory work through logistics support and coordination of field activities as well as in post-activity work (documentation, safekeeping of equipment, and archiving of files). He/She will also be supporting the Public Health Officer in field monitoring, i.e. collecting the data and maintaining/updating the project database. implementing, monitoring and evaluating the ‘Increasing access to quality and inclusive diabetes care project’ in 3 districts/city; Sleman, Yogyakarta and Gunungkidul. The tasks in particular are:

•    Support to carry out capacity-building activities of primary health care professionals and community health volunteers (Posbindu cadres) on diabetes care including screening and health promotion on diabetes
•    Support to coordination of partners
•    Support to project follow-up

DUTY STATION: Community Health Facilitator will be based in Yogyakarta

KNOWLEDGE REQUIREMENTS:

* Degree in any health-related disciplines or social sciences. Degree in nursing or public health is preferred
* 1-2 years experience in events organization and/or training delivery is preferred
* Experience working with INGO is an advantage
* Experience in and ability to work effectively with people from diverse backgrounds
* Good professional or personal knowledge of diabetes is an advantage
*Computer skills especially MS Word, Excel, Publisher and Powerpoint and online collaboration tools (e.g. Google Drive) is an advantage
* Passive English and Javanese Language a must
* Good oral and written communication skills

Personal Qualities:
*Able to be part of a small, collaborative work team and possesses leadership skills
* Ability to work independently
*Good problem solving and stress management skills

Miscellaneous:
*Licensed  and willing to ride a motorbike for work purpose
*Local recruitment – there is no support for mobility for non-resident applicants out of Yogyakarta

Initial contract will be for maximum 1 year with a possibility for renewal until the end of the project.

Job description: Community Health Facilitator

Under the responsibility of the Diabetes Project Manager, the Community Health Facilitator will be engaged in the implementation of the project ‘Increasing Access to Quality & Inclusive Diabetes Care.” The project has four intended results:
•    Reduce diabetes related disabling complications by increasing access to quality, integrated, comprehensive and inclusive diabetes capacity buildings.
•    Services for prevention and diabetic foot care using the step-by-step model are available in the projects’ area of interventions as an integrated component.
•    Contribute to the creation of an enabling policy environment at local level for the integration of the prevention, early detection and management of the diabetic foot into existing health services.
•    Project impacts and results are monitored, evaluated and shred across local stakeholders in the province of Yogyakarta.

Duty Station: The Post holder will be based in Yogyakarta
Duration of contract: 15 months with the expected starting date: early October 2019

Stakes of Position:

Under the direct line management of the Project Manager, the Community Health Facilitator is responsible in supporting preparatory work through logistics support and coordination of field activities as well as in post-activity work (documentation, safekeeping of equipment, and archiving of files). He/She will also be supporting the Public Health Officer in field monitoring, i.e. collecting the data and maintaining/updating the project database.  
Essential Job Duties/ Scope of Work
1. Support to Public Health Officer to carry out capacity-building activities of primary health care professionals and community health volunteers (Posbindu cadres) on diabetes care including screening and health promotion on diabetes.

a.    Provides support during preparation and implementation of project activities such as baseline and endline surveys, on the job training of public health care professionals, training for community volunteers, coordination with project partners and documentation of project activities
b.    Supports in facilitating the trainings, if necessary
c.    Participates in the review and improvement of the training designs and methodology
d.    Assists in the encoding of training presentations, manuals, references, and other IECs
e.    Assists in the preparation of all training presentations, kits and other logistics
f.    Assists in the implementation of all trainings
g.    Follows up post-training documentation from partners, as necessary  

2. Support to coordination of partners
a.    Assists in the coordination of capacity-building activities of health care professionals and community health volunteers  and other relevant activities through:
-    Support in the identification of participants
-    Preparation of invitation letters, dissemination, and follow-up
b.    Works with the local health authorities in the conduct of trainings and other project activities, including coordination and finalization of schedules and venues, and identification of focal persons for each of the trainings among others.

3. Support to project follow-up
a.    Participates and contributes in the development of project technical reports and other documents
b.    Participates in project and partner meetings
c.    To be the person in charge for the administration and logistic needs related to project activities
d.    Participates in the regular field monitoring and supports in data collection
e.    Manages the photo library, project database, donation certificates, attendance sheets, digital archive, and other project documents
f.    Performs such other functions and duties as the operation or exigencies of the project may require
