Background

Tuberculosis (TB) is a major health problem in Indonesia. Although there has been a 19% mortality decline from 2007 to 2017, TB remains the leading cause of death by communicable diseases. The Government of Indonesia has adopted the global commitment to end TB by 2030, and now must decide what to prioritize and how to deliver. From 2017 – 2020 the Center for Financing and Health Insurance (PPJK) of Ministry of Health (MoH) led analytical works of multi-stakeholders (Tim perumus) including development of TB Strategic Health Purchasing (SHP) pilot design.

The Tim Perumus outlined priority service delivery objectives for TB and identified health financing bottlenecks to achieving those objectives. The SHP team supported analysis, formulation and validation of options, and developed recommendations for achieving better TB outcomes using more strategic health purchasing. After finalizing the recommendations, the SHP team looks forward to conducting pilot preparation and implementation in the selected districts from 2020 to 2023. During the design and implementation preparation (ToT on SHP governance manual) period, an observation will be conducted to capture stakeholder concerns and respond to them. It includes gathering insights for the further implementation arrangement.

The Director of Communicable Diseases of the Ministry of Health had selected two pilot districts for TB SHP which are Kota Medan, North Sumatra and Kota Denpasar, Bali. Pilot preparation in Denpasar began in January 2021 and should be completed by the end of Q3 2021. Adjustment to the contracting arrangements are expected to come first at Q4 2021, followed by the implementation of new payment arrangements in 2022.

The design and implementation of TB SHP are supported by two USAID projects – The Health Financing Activity and the TB for Private Sector (TBPS) project.  The Health Financing Activity is responsible for leading design, national coordination and analytical works, and providing technical input to the development of modules related to capacity building and advocacy in regional level. TBPS is responsible for initial assessment at district level, district coordination and facilitation of the SHP pilot process at district level, including personnel support, data collection, and coordination with district authorities related to SHP works.

This Technical Officer Consultant will be working under USAID TBPS to facilitate the TB SHP pilot in Kota Denpasar, Bali. The consultant will be working closely with USAID TBPS’ District Technical Officer (DTO) for Denpasar in coordination with USAID HFA. This consultant will be administratively responsible to the Technical Monitor from FHI 360, but the day-to-day manager will be the Health Financing Officer from R4D.

Scope of Work

Key activities Technical Officer Consultant for SHP Pilot at each District*

  1. Lead outreach to key stakeholders at the district level

a.       Facilitate project engagement with key stakeholder groups, including professional associations

b.       Provide technical and coordination support to SHP team including meeting organization, drafting        agendas and materials as needed

c.        Gather relevant information during the technical design development of a Strategic Health                   Purchasing pilot for TB health services

d.       Liaise with partner organizations and project counterparts to coordinate, align, and build on                   analytical and technical input to SHP processes and health financing objectives

  1. Contribute to technical design elements of the pilot manual

a.      Review and propose additions and/or revisions for sections of the technical implementation                 manual (SoP) development on purchasing pilot arrangements overall and adapted for each                 district, including services purchased through APBN and APBD mechanism;

  1. Support pilot implementation activities including training and monitoring

a.         Assess sub-national (district and sub district) capacity and needs for improving enabling factors           needed for pilot implementation (e.g. strengthen monitoring mechanism of providers, improving           quality service delivery by improving providers capacity, strengthening planning referral                       mechanism, and improving providers capacity in case finding and notification);

b.         Provide support to key stakeholders (e.g. providers, Dinkes, BPJSK, etc) to improve education,           quality of services, and networking of providers as required by the design of the pilot;

c.          Establish routine checks and feedback mechanisms to assess understanding and address         challenges of implementers

d.       Communicate results orally and in writing to a variety of audiences

Key Qualifications

  1. Clinical and/or public health background - degree in medicine, public health, nursing, pharmacy, health economics or policy
  2. 3-5 years of direct work experience related to health financing and related fields
  3. Fluency in oral and written English and Bahasa Indonesia
  4. Sound knowledge of the Indonesian health system, Ministry of Health, BPJS-K, DJSN, or other health sector.
  5. Analytical, problem solving, and quantitative skills, with experience in analytical tools for health financing (costing, expenditure tracking, etc.).
  6. Research experience with data collection and leading FGDs and in-depth interviews
  7. Ability to conduct virtual meetings through online platforms
  8. Strong experience in design and implementation of aid development program at sub national level is desirable
  9. Experience in building stakeholder relationships and facilitating decision-making at district level to create positive change
  10. Strong experience in facilitating technical workshops, including developing agendas and background materials, presenting to and facilitating discussion
  11. Ability to work independently and collaboratively as part of a team and deliver high- quality results within tight timeframes.
  12. Willingness for up to 25% domestic travel
